國人的肥胖盛行率有逐年增加之趨勢,而肥胖與許多慢性疾病有關,另肥胖也是代謝症候群的主要原因,而減重被建議為第一線之治療,因此本研究希望藉由體重控制計畫介入後,探討體重過重者及肥胖者減重後,身體組成與血液生化檢驗值之改變狀況。
研究對象,只選取體重過重者( BMI≧24(kg/m2)) 及肥胖者(BMI≧27(kg/m2)),進行為期十二週之體重控制計畫。本研究之體重控制計畫採三合一控制計畫,治療方式包括耳針治療配合飲食及運動指導,經十二週治療後,探討身體組成(包含體重、身體質量指數(BMI)、體脂肪百分比、體脂肪重量、腰圍、腰臀比 )、血液生化檢驗值(包含飯前血糖、血清總膽固醇、低密度脂蛋白膽固醇、高密度脂蛋白膽固醇、三酸甘油脂、肝功能、腎功能、尿酸及血色素)之改變狀況。
研究結果發現:全部個案369位,經十二週治療後,體重平均減少13.9±5.0㎏;身體質量指數(BMI)平均減少5.3±1.8 kg/m2;體脂肪百分比平均減少8.8±2.5%;腰圍平均減少12.8±5.8 公分;腰臀比平均減少0.03± 0.04;空腹血糖平均減少5.0 ±12.0 mg/dl;總膽固醇平均減少30.1 ± 26.2 mg/dl;低密度脂蛋白膽固醇平均減少31.2± 24.8 mg/dl ;高密度脂蛋白膽固醇平均增加 7.4 ± 7.8 mg/dl;三酸甘油脂平均減少 24.5 ± 52.1 mg/dl;胺基丙酮酸轉氨&#37238;(GPT)平均減少16.1 ± 24.3 mg/dl;尿酸(Uric acid)平均減少1.0 ± 5.5 mg/dl,而血色素(Hb)平均減少 0.5 ± 0.6 g/dl;因此,研究對象在身體組成方面(體重、身體質量指數(BMI) 、體脂肪百分比、腰臀圍比) 及血液生化檢驗方面 (空腹血糖、血清總膽固醇、低密度脂蛋白膽固醇、高密度脂蛋白膽固醇、三酸甘油脂、胺基丙酮酸轉氨&#37238;(GPT)、與血色素),其前測與後測間是有顯著差異存在的( P<0.001),唯獨在腎功能(肌酐酸 Creatinine) 是沒有顯著差異的。
結論:故十二週之體重控制計畫是可以做為肥胖個案一級及二級之預防保健諮詢與健康促進的衛教教材,也可以做為未來肥胖個案治療模式的參考。
Abstract
Obesity is a significant issue for health care systems over the world, including Taiwan. In particular, obesity is a chronic disease with monumental public health implications, which associated with metabolic syndrome, and weight reduction was suggested to first line treatment.
The objective of the study is to explore the efficacy (including of body composition and blood biochemical lab data) after ear acupuncture combined therapy with diet and physical activity .
Participants and Setting: 369 people which belong to overweight ( BMI≧24 kg/m2) and obesity (BMI≧ 27kg/m2 ) and a 12-week body weight control trial (purposive sampling trial) from July to December 2006. Intervention: Ear acupuncture was treated weekly for 12 weeks and participants need to keep their regular daily diet and physical activity. Our objective is to explore the efficacy (including of body composition and blood biochemical laboratory data) after 12-week body weight control trial.
Results: Participants and Setting: 369 people and a 12-week body weight control trial from July to December 2006, the average of body weight for participants reduction 13.9 ± 5.0 Kg (p<0.001). BMI for participants reduction 5.3 ± 1.8 (p<0.001). Body fat for participants reduction 8.8 ± 2.5 % (p<0.001). Waist circimference for participants reduction 12.8 ± 5.8 cm (p<0.001). Waist to hip ratio for participants reduction 0.03 ±0.04 (p<0.001).Fasting sugar for participants reduction 5.0 ± 12.0 mg/dl (p<0.001). Total cholesterol for participants reduction 30.1 ± 26.2 mg/dl (p<0.001). LDL-cholesterol for participants reduction 31.2 ± 24.8 mg/dl (p<0.001). HDL-cholesterol for participants increasing 7.4 ± 7.8 mg/dl (p<0.001). Triglyceride for participants reduction 24.5 ± 52.1 mg/dl (p<0.001) GPT for participants reduction 16.1 ± 24.3 mg/dl (p<0.001). Uric acid for participants reduction 1.0 ± 5.5 mg/dl (p<0.001). But hemoglobin for participants reduction 0.5 ± 0.6 g/dl (p<0.001).
Conclusion: These findings indicate that 12-week body weight control trial can be effective in treating obesity.
