過去曾有研究指出,冷凍肩病人之肩峰下滑囊中,組織的發炎程度和肌纖維母細胞數量與肩部僵直與否有關。但發炎性細胞激素與肌纖維母細胞間交互作用導致肩關節僵直之機轉至今仍未充分研究。本論文旨在探討 Interleukin-1 β(IL-1 β)對於來自轉肌袖疾患合併肩部僵直病人之肌纖維母細胞之影響。將取自病人肩峰下滑囊的肌纖維母細胞做成初代細胞後,加入或不加IL-1 β培養。另外,也收集肩部有或無僵直病人之肩峰下滑囊組織。然後利用Real time RT-PCR與免疫染色來觀察細胞與病人組織上的α-smooth muscle actin(α-SMA)和prolyl 4-hydroxylase(PH-4)表現。結果顯示,加入IL-1 β培養的肌纖維母細胞其PH-4的表現明顯高於未加IL-1 β的細胞,然而α-SMA的表現則低於未加入IL-1 β培養的細胞;肩部僵直之病人組織中PH-4的基因表現亦明顯增加,但是α-SMA的基因表現則無明顯差異。根據上述實驗結果,我們發現IL-1 β會讓細胞中PH-4的表現增加、α-SMA的表現下降。這代表肌纖維母細胞因IL-1 β的影響而轉變成纖維母細胞。
Abstract
Previous investigations have indicated that shoulder stiffness was related with the degree of inflammation and the number of myofibroblasts in subacromial bursa tissue of rotator cuff lesions. But the interaction mechanism between inflammatory cytokines and myofibroblasts induced shoulder stiffness has not been fully explored. The purpose of this research is to study the effect of IL-1 β on myofibroblasts in rotator cuff lesions with shoulder stiffness. Myofibroblasts from subacromial bursa tissue of rotator cuff lesions were cultured with or without IL-1 β. The gene and protein expression of α-smooth muscle actin (α-SMA) and prolyl 4-hydroxylase (PH-4) were determined. The results showed that high levels of PH-4 and low levels of α-SMA were expressed in myofibroblasts cultured with IL-1 β. PH-4 gene had higher expression in tissue of stiff patients than non-stiff patients, but α-SMA gene expression had no significant difference. According to the results, myofibroblasts cultured with extra IL-1 β showed increased PH-4 expression and decreased α-SMA expression. This indicates myofibroblasts might transdifferentiate into fibroblasts via IL-1 β effect.
